New Patients

We are currently NOT accepting new patients. Our waitlist is FULL as of June 18, 2024.

If you are a part of our clinic’s roster, please follow the steps below to complete your patient profile.

  1. Complete your Intake Form within 7 days of receiving your intake email
  2. Once we have received and reviewed your form, we will contact you for a TELEPHONE Intake Review
  3. Prior to your telephone appointment, complete your Patient Agreement and sign up for the Patient Portal
  4. Schedule and complete your Intake Physical
  5. This process must be completed in 1 month otherwise your file will be returned to the attachment coordinator, with the assumption you are not interested in joining our clinic. This is to allow us to efficiently work through our wait list.
