To send a document or form to you Family Physician, please follow the steps below:
1. Ensure that the document or form is saved as a PDF file.
2. Compose an email and attach the PDF file.
3. In the subject line of the email, please include your Last Name, First Name, followed by either your Personal Health Number or Date of Birth.
4. Send the email to contact@nimbusmedical.ca.
5. Please use a secure way to send your file to us. As per government guidelines, we utilize a secure email under the PIPA (Personal Information Protection Act). If there are any privacy concerns about the method you are sending your files, please bring your printed paperwork in person to upload it to your file.Please ensure that all information is accurate to avoid any delays or confusion.

If you are unable to attend your appointment or are running late, please contact our clinic via telephone or email. Please provide us 24 hours notice to avoid the No-Show/Late Cancellation/Late Attendance Policy. This will allow the appointment slot to be utilized by another patient. Out of respect of everyone’s time, we may not be able to accommodate you if you are late and you will be asked to rebook.
Each appointment time is scheduled for 15 minutes, this includes the pre-appointment measurements, patient-physician interaction and paperwork preparation(referrals, charting, prescriptions, forms). We do request that you arrive 5 minutes prior to your set appointment time to allow for any pre-appointment measurements that may be required for your appointment.
Please note that any late cancellations, no-shows and late attendance will result in the No-Show fee being charged.
