New Patients

We are currently only accepting new patients through the Health Connect Registry.
Please register yourself on the BC government website and you will be connected with the next available clinic. 

If you have received an email from our clinic, please follow the steps below to complete your patient profile. 

  1. Complete your Intake Form within 7 days of receiving your intake email
  2. Once we have received and reviewed your form, we will contact you for a TELEPHONE Intake Review
  3. Prior to your telephone appointment, complete your Patient Agreement and sign up for the Patient Portal
    4. We respect your rights to disagree to our attachment terms, however, we are unable to deviate from these terms due to clinic efficiency and patient safety. Your file may be sent to the attachment coordinator in the hope of finding you a better suited clinic.
  4. Schedule and complete your Intake Physical
  5. This process must be completed in 1 month otherwise your file will be returned to the attachment coordinator, with the assumption you are not interested in joining our clinic. This is to allow us to efficiently work through our wait list.
